Options for Everyone

Sysco provides you with four medical plan options so you can select the one that’s right for you and your family:

• Basic Plan
• HSA Plan
• PPO Plan
• National HMO Plan

All four plans are administered by Aetna and you have access to their broad national network of providers. Use the What’s Your Medical Type quiz to help you choose the plan that’s best for you. Once you choose your plan, you can select the level of coverage you need, shown in the Levels of Coverage box.

The cost of coverage can be found in the Total Rewards Café.

Register on Aetna.com

As a first-time user, you will need to register for an account. Click here or go to Aetna.com and click on the purple Login button on the right hand side of the page.

To register for an account, click on Register on the bottom of the login page. You can find your Member ID on the Aetna ID card that was mailed to your home address on file with Sysco. If you don't want to use your Member ID, you can use your Social Security number. Then fill in the rest of the requested information and register for your account.

Find an Aetna Provider

Click here to see if your provider is in the Aetna network. You will be prompted to select a plan name. The plan names will include the medical plans below:

• Basic/PPO/HSA Medical plans – Aetna Choice POS II network
• National HMO Medical plan – Aetna Select Open Access network
• Basic and Premium Dental plans – Aetna Dental PPO Network

24-Hour Nurse Line

As part of your Aetna medical plan, you have access to the 24-Nurse Line. Call the 24-Hour Nurse Line to:

• Get information on a wide range of health and wellness topics
• Find out more about a medical test or procedure
• Get help preparing for a doctor visit
• Receive emails with links to videos that relate to your question or topic

The 24-Hour Nurse Line is available 24/7 and at no cost to you if you are enrolled in a Sysco-sponsored Aetna national medical plan.

We’re Self-Funded

Sysco’s medical plans are self-funded, meaning that while claims are processed and paid by companies like Aetna, the money actually comes from Sysco’s bank account. Sysco funds the majority of the cost and colleagues fund the rest through premiums, deductible expenses, copays, and coinsurance. Throughout the year, we will encourage you to use these benefits as wisely as you use other Sysco resources. Using generic drugs instead of name brands, getting regular preventive care, and creating a relationship with a good doctor are all ways to use your medical benefits wisely.

Finding Doctors & Other Medical Providers

You’ll get the best cost for care if you see a network provider. Find an Aetna network doctor or facility.

If you choose the National HMO plan, you must use Aetna network providers to receive any benefits. If you choose one of the other plans, using network providers will help you get the best benefit. Note that the National HMO plan is not available in all areas.
